What a testosterone blood test costs in Australia and New Zealand

There are three main ways to get your testosterone checked in Australia and New Zealand: through your GP, by ordering a private blood test online, or with a saliva kit at home. They differ in price, in which markers come back on the report, and in how useful the result is when you take it to your GP. Here is what each route costs, with a source for every number.

How much does a testosterone test cost in Australia?

If your GP requests the test and both the practice and the lab bulk bill, it costs you nothing. A private testosterone test ordered online costs A$53 to A$86, and you pay the full price yourself.

At Medicare's schedule fees, the GP visit costs more than the test. Medicare pays A$45.05 for a standard consult lasting at least 6 minutes and under 20 minutes (item 23) and A$87.10 for one lasting at least 20 minutes (item 36), at the fees that applied from 1 July 2026³¹². A bulk-billed visit costs you nothing. Otherwise you pay the practice's fee, claim the rebate, and the difference is your gap¹³. Since 1 November 2025, GPs can claim bulk billing incentives for every Medicare-eligible patient they bulk bill, not only children under 16 and concession card holders¹⁴. Each practice still chooses whether to bulk bill, so ask when you book¹³.

Is a testosterone test bulk billed in Australia?

It can be, when your GP requests it as part of looking into symptoms. Medicare covers most pathology tests, and if your test is bulk billed, Medicare pays the full cost and you pay nothing⁵. For Medicare to pay, there has to be a medical reason for the test⁵, and Medicare rules class a test that isn't reasonably necessary for your care as excessive¹⁵. The Endocrine Society of Australia says testosterone testing is not warranted for general screening¹⁶. So whether you get a Medicare-funded test is your GP's call. Describe what you've noticed honestly, without playing it up or down.

You can ask your GP to select the bulk bill option on the pathology request form, and ask the lab whether there will be any out-of-pocket costs⁵. Private online tests sit outside Medicare and you pay the full price. MediTests, for example, emails results to you and doesn't link them to Medicare or My Health Record⁶, and i-screen doesn't ask for a Medicare card⁸. If anything on the report needs follow-up, you take it to your GP yourself.

How much does a testosterone test cost in New Zealand?

If your GP requests it to investigate symptoms and you're eligible for publicly funded care, the lab test is usually free and you pay only for the GP visit. In Auckland, for example, funding applies when the test is on the regional schedule, you meet the eligibility rules and it's requested for diagnosis¹⁷. bpac nz notes that testosterone testing isn't routinely needed in men without symptoms, and that some regions may restrict free testosterone testing, for example funding it only when total testosterone is inconclusive (7 to 15 nmol/L) or an endocrinologist recommends it¹⁸. Self-requested tests are not funded¹⁹, and neither is wellness testing of a person with no symptoms or risk factors²⁰.

Lab fees vary by region. Awanui in Wellington charges a NZ$22 administration fee per visit for self-requested tests³¹, and Canterbury Health Laboratories, which only accepts tests with a clinical referral, lists a non-funded testosterone test at NZ$34.83 plus GST³²³³.

GP fees vary too. General practices are private businesses and set their own fees. Enrolled patients usually pay less than the casual rate, and your local primary health organisation's website lists practices and their fees³⁴. Most practices also offer lower-cost visits to Community Services Card holders³⁵. For 2026/27, Health NZ has held standard GP consult fees at the level set on 8 June 2026, and practices can't raise them unless Health NZ approves an exception³⁶. As one example, the THINK Hauora list for the MidCentral district shows fees of NZ$19.50 to NZ$67 for adults aged 25 to 44 at most of its practices³⁷.

Can you get a free testosterone test in NZ?

Yes, if you mean no cost: a GP-requested test for symptoms is usually funded for eligible patients, as above. If you mean free testosterone, the fraction not bound to proteins, Awanui, CHL and Pathlab all report a calculated figure, and they only calculate it when SHBG is measured alongside total testosterone³⁸³²²². So the wording on the form matters.

Why doesn't my blood test show free testosterone?

Usually because SHBG wasn't measured, and the lab's request options decide that. Since 17 September 2024, Awanui Labs has offered four testosterone options³⁸:

Awanui also asks for samples between 8am and 10am³⁸. Canterbury Health Laboratories (CHL) no longer accepts self-requests, so every test there needs a clinical referral³³. CHL asks for a morning sample before 10am, calculates free testosterone from total testosterone and SHBG using the Vermeulen equation, and reports it in pmol/L rather than nmol/L³². If your report uses different units, such as ng/dL, our unit converter lines them up. Pathlab MyWay's product pages say free testosterone is reported when testosterone and SHBG are both requested²².

What does each option actually measure?

The cheapest options usually give you total testosterone only. Total testosterone counts the hormone bound to SHBG and albumin as well as the free fraction, which bpac puts at only 1 to 2% of the total¹⁸. It is the test most often ordered, and a doctor may add SHBG when an unusual SHBG level could skew the total³⁹. Free testosterone is then calculated from total testosterone and SHBG, and the Vermeulen equation also uses albumin⁴⁰. That's why a total-only test can't give you a free testosterone figure, however cheap it is.

How much weight that calculated figure deserves is where the guidance differs. The Endocrine Society of Australia says SHBG can be informative but calculated free testosterone is not recommended for clinical decisions¹⁶. bpac nz, which writes guidance for New Zealand GPs, says direct free testosterone immunoassays are unreliable and prefers a calculated value if doubt remains after two morning tests¹⁸. The US Endocrine Society guideline recommends a free testosterone value, measured by equilibrium dialysis or estimated with an accurate formula, when total testosterone is near the lower limit or a condition alters SHBG⁴¹. Guidance also recommends measuring LH and FSH alongside testosterone, and bpac adds prolactin when confirming a low result¹⁶¹⁸. Through your GP, they can go on the same request form. If you're paying privately, they add up:

Why might you end up paying for two tests?

Because one low result isn't enough to go on. Testosterone is highest in the morning, so the Endocrine Society of Australia recommends a sample between 8am and 10am and notes that fasting gives higher readings¹⁶. bpac nz gives a 7am to 11am window, ideally after an overnight fast, and says a result below the range should be repeated, preferably four weeks later, while a normal result doesn't need repeating¹⁸. Up to 30% of men with an isolated low result are normal on the repeat test, which is why, unless the first result is clearly conclusive, the ESA recommends at least two measurements of testosterone, LH and FSH before a diagnosis¹⁶. The US guideline also asks for a repeat morning fasting test⁴¹. If you work shifts, the ESA suggests testing shortly after waking instead¹⁶.

Fasting instructions vary by lab and test. healthdirect says you usually need to fast⁴⁴. The total testosterone tests from BODYiQ and Bloody Good don't require it, while BODYiQ's SHBG panel asks for an 8 to 12 hour fast²⁵⁹²⁷. Follow the instructions that come with your form.

So if your first result comes back low, budget for a second test. Two private tests that include SHBG cost A$136 to A$172 in Australia, or NZ$160 at Pathlab MyWay and NZ$209.98 through BODYiQ, before any GP visit or collection fee^6710222327. Use the same lab for the repeat if you can. Reference ranges depend on the lab and the method: CHL's range for men aged 15 to 50 is 8.6 to 29 nmol/L on an immunoassay³², while the ESA quotes 10.4 to 30.1 nmol/L for men aged 21 to 35 with normal reproductive function, measured by mass spectrometry, and notes that immunoassay ranges vary between methods¹⁶. Are saliva or home testosterone kits worth the money?

If you want a result your GP can use, a blood test is the better buy. Australian, New Zealand and US guidance all describe testing with a morning blood (serum) sample¹⁶¹⁸⁴¹. The saliva kit we found in Australia costs A$139¹¹, more than a private blood test that includes SHBG. One 2006 validation study found saliva testosterone tracked blood measures well enough to work as a screen, but concluded it was not a better test for diagnosis⁴⁵. That is a single study, and the guidance above hasn't adopted saliva. If your GP then asks for a blood test, you've paid twice.

Some broader hormone bundles also include PSA, a prostate test, such as BODYiQ's male hormone bundle at NZ$521.99⁴⁶. The Prostate Cancer Foundation of NZ notes that PSA tests are now sold directly to the public, advises talking with a health professional before any PSA test, and does not endorse home PSA testing⁴⁷. If a bundle you're looking at includes PSA, talk to your GP before you order it.

What's the cheapest sensible way to get tested?

For most men, start with the GP. Then:

1. Say why you're there. Describe what you've noticed and ask whether the test will be bulk billed (Australia) or funded (New Zealand). Take our blood test checklist, and if you're over 40, our guide to blood tests for men over 40.
2. Ask whether SHBG will be measured. Without it there's no free testosterone figure. At Awanui, that's the "Testosterone + SHBG" option, not "Testosterone only"³⁸. In New Zealand, funding for it can depend on your first result¹⁸.
3. Book the collection for early morning and follow the fasting instructions on the form.
4. If you pay privately, compare prices with and without SHBG. Adding it costs A$33 more at Bloody Good and NZ$40 more at Pathlab MyWay⁹¹⁰²³.
5. Keep the PDF. A result outside the range goes to your GP. If a repeat is needed, using the same lab makes the comparison fair. Our safety page lists what should go straight to a doctor.
